About this notice

This Notice of Privacy Practices describes, in general terms, how New Frontier Recovery Center may use and disclose protected health information about you and how you can get access to that information. Protected health information is information that identifies you and relates to your health, treatment, or payment for care.

This is a general notice provided for transparency. It is distinct from the privacy policy that governs this website. New Frontier is required by law to maintain the privacy of protected health information and to provide you with notice of our legal duties and privacy practices.

How protected health information may be used and disclosed

In general, and consistent with applicable law, protected health information may be used and disclosed for purposes of treatment, payment, and health care operations. Treatment means coordinating and providing your care. Payment means activities needed to obtain reimbursement for care, including verifying benefits. Health care operations means the internal activities needed to run the facility and support quality of care.

In some situations, the law permits or requires disclosure of protected health information without your authorization, for example to comply with legal requirements or to respond to a serious threat to health or safety. Substance use disorder treatment records also carry additional federal confidentiality protections, and we handle them accordingly. Other uses and disclosures generally require your written authorization, which you may revoke.

Your rights

You have rights regarding the protected health information we maintain about you. In general terms, and subject to applicable law, these include the right to request access to and a copy of your information, to request corrections, to request restrictions on certain uses and disclosures, to request confidential communications, and to receive an accounting of certain disclosures. You also have the right to a paper copy of this notice on request.
